Indications & Dose: DYSLIPIDEMIAS Mono therapy/Adjunct therapy PO Adult Initially 250mg OD may increase to 1.5-2g/day in 2-3 divided doses, after 2 months further increased to 3g/day in 3 divided doses at 2-4 wk intervals max 6g/day, Sustained release: 1-2g/day, ER: Initially 500mg HS 4wk then 1g HS for next 4 wk, increase dose by 500mg/day q4wk to 2g/day based on response & tolerabilityFredrickson types IIa & IIb or primary hypercholesterolemia | PELLAGRA PO Adult 50-100mg TID/QID max 500mg/day Child 50-100mg/dose TID |
| Contra: Hypersensitivity, active hepatic disease, significant/unexplained persistent elevations in hepatic transaminase, active PUD, arterial hemorrhage
Precautions: History of PUD, DM, gout, do not substitute modified release tablet with IR formulation, CVD, hepatic/renal impairment, gall bladder disease ADR: Serious: Arrhythmias, atrial fibrillation, toxic amblyopia, hypersensitivity reactions, PUD, flushing, hypotension, tachycardia, gout, platelet counts decreased, hepatotoxicity, increased PT, blurred vision, myopathy, Others: paresthesia, cough, dyspnea, diaphoresis, increased LDH, leg cramps, myalgia, myasthenia, decreased glucose tolerance, nausea, vomiting, chills, dizziness, headache, insomnia, migraine, nervousness, pain, dry skin, maculopapular rash, pruritus, skin discoloration, orthostasis, palpitation DDI: Serious Statins causes additive muscle toxicity, Antidiabetics causes deterioration in glucose tolerance, Telbivudine causes myopathy, Alcohol leads to flushing, pruritus & possibly liver toxicity Diet: With food Monitor: Blood glucose (in DM), CPK & serum K, LFT, lipid profile, platelets, PT, uric acid, phosphorus |
