COUMARIN DERIVATIVE | ANTI-COAGULANT
PK: A: Rapid D: 0.14L/kg (Vd) M: Hepatic E: Urine

Indications & Dose: THROMBOEMBOLIC DISORDERS PO Adult Treatment/prophylaxis: 10mg/day (as induction dose) 2 days, adjust according to patient response & subsequent dose depends upon PT MD 3–9mg/day given at same time each day

Contra: Hypersensitivity, hemorrhagic disorders, hemophilia, blood dyscrasias, thrombocytopenia purpura, recent surgery, head injury, spinal puncture, malignant HTN, threatened abortion, history of warfarin-induced necrosis, aneurysm, alcoholism, unsupervised senile/psychotic patient, subacute bacterial endocarditis, an unreliable, noncompliant patient, eclampsia/pre-eclampsia, patient who has a history of falls/is a significant fall risk

Precautions: CHF, heparin-induced thrombocytopenia, moderate-to-severe renal impairment, thyroid disease, hepatic impairment, arterial puncture, occult blood in stool/urine, patients with acute infect/active TB/any disruption of normal GI flora

ADR: Serious: Hemorrhage, drug-induced necrosis, cholesterol microembolism, angina, hypotension, syncope, vasculitis, osteoporosis, agranulocytosis, anemia, leukopenia, retroperitoneal hematoma, Others: Nausea, vomiting, diarrhea, abdominal cramps, stomatitis, anorexia, hematuria, rash

DDI: Serious Amiodarone/Allopurinol/Azoles/Chloramphenicol/Disulfiram/Fibrates/Fluorouracil/Heparin/Isoniazid/Quinolones increases drug effects, Carbamazepine/Aminoglutethimide/Glutethimide/Griseofulvin/Mesalazine/Rifampicin reduces drug effects

Diet: With/without food

Monitor: PT, hematocrit, INR
