Indications & Dose: CLUSTER HEADACHE SC Adult 6mg single dose, if no response after 10-15min next dose 6mg after 1h, max 12mg/dayIf no response with initial dose second dose should not be given for same attack | MIGRAINE Acute PO Adult 50mg or 100mg, second dose after 2h interval, max 300mg/day Child 6-10 yr: 25mg; 10-12 yr: 50mg, repeat once after 2h; >12 yr: same as adult doseIf no response with initial dose second dose should not be given for same attack | Acute SC Adult 6mg single dose, if no response after 10-15min give next dose 6mg after 1h, max 12mg/dayIf no response with initial dose second dose should not be given for same attack | Acute NSL Adult 5-20mg single dose into one nostril, may repeat after 2h interval, max 40mg/day Child 12-17 yr: 10mg into one nostril, the dose may be repeated after at least 2h if symptoms recur within 24h, max 20mg should be used within a 24hIf no response with initial dose second dose should not be given for same attack |
| Contra: Hypersensitivity, IHD, cerebrovascular syndromes, peripheral vascular disease, uncontrolled HTN, hemiplegic/basilar migraine, severe hepatic impairment, MAOIs, 5-HT1 agonists/ergotamine- containing drugs
Precautions: CVD, elderly, children <18 yr ADR: Serious: chest discomfort/tightness/pressure, coronary vasospasm, MI, Others: myalgia, Dizziness, warm/hot sensation, Inj site reaction, paresthesia DDI: Serious SSRIs causes dyskinesia, Ergotamine causes vasoconstriction, Moclobemide increases drug bioavailability, Lithium leads to serotonin syndrome, Methysergide results in MI |
