Indications & Dose: CYSTITIS Uncomplicated PO Adult 500mg q12h for 1-2wks Child >15yr: 500mg q12h for 1-2wks | ENDOCARDITIS Prophylactic therapy PO Adult 2g 30-60 min before procedure Child >1yr: 50mg/kg 30-60 min before procedure, max: 2g; >15yr: 2g 30-60 min before procedure | FURUNCULOSIS PO Adult 250mg QID Child >1yr: 25-50mg/kg/day in 4 divided doses; >15yr: 250mg QID | MILD-TO-MODERATE RTI PO Adult 250mg q6h Child >15yr: 250mg q6h | OTITIS MEDIA Acute PO Child 75-100mg/kg/day in 4 divided doses | SUSCEPTIBLE INFECT PO Adult 1-2g/day in divided doses at 6, 8, 12h interval, in severe or deep seated infect may be increases to 6g/day Child 25-100mg/kg/day in divided doses, max 4g/day |
