Indications & Dose: CHICKENPOX PO Child 2-17 yr: 20mg/kg/dose TID 5 days, max 1g TID | GENITAL HERPES PO Adult 1gm BID 10 days for initial episode, 500mg BID 3 days for recurrent episodes, 500mg/day is taken by the infected partner to reduce transmission | HERPES ZOSTER PO Adult 1g TID 7 days | SUPPRESSION OF HERPES SIMPLEX INFECT PO Adult 500mg/day in 1-2 divided doses (immunocompetent) |
Contra: Hypersensitivity, pediatrics
Precautions: Renal impairment, elderly ADR: Serious: neutropenia, increased LFTs, thrombocytopenia, herpes simplex, hypersensitivity reactions, delirium, erythema multiforme, TTP, tremor, Others: agitation, alopecia, arthralgia, rhinorrhea, headache, nausea, nasopharyngitis, fatigue, depression, dizziness, rash, dysmenorrhea, vomiting, diarrhea DDI: Serious Cimetidine increases AUC of valaciclovir, Mycophenolate neutropenia, Probenecid reduces renal excretion & increases valaciclovir plasma levels Diet: With food Monitor: Urinalysis, BUN, SeCr, LFTs, CBC |
