Indications & Dose: INDUCTION OF GENERAL ANESTHESIA IV Adult (<55 yr): 2-2.5mg/kg (~40mg q10sec until onset of induction) Child 2.5-3.5mg/kg over 20-30sec Elderly 1-1.5mg/kg (~20mg q10sec until onset of induction) | MAINTENANCE OF GENERAL ANESTHESIA IV Adult (<55 yr): Initially 100-200µg/kg/min as inf over 10-15min, decrease by 30-50% during first 30min of maintenance Child 2 month-16 yr: Initially 200-300µg/kg/min, may decrease inf rate after 30min Elderly 50-100µg/kg/min | MONITORED ANESTHESIA CARE SEDATION IV Adult (<55 yr): 100-150µg/kg/min as inf over 3-5min Elderly 80% of adult dose | Maintenance therapy IV Adult (<55 yr): 25-75µg/kg/min as inf or incremental bolus dose of 10mg or 20mg Elderly 80% of adult dose |
Contra: Hypersensitivity to drug/any component of formulation, hypersensitivity to egg products, soya products, sedation in child
Precautions: Anaphylaxis, hypertriglyceridemia, CVD, pancreatitis, patients with IOP, respiratory disease, history of epilepsy/seizures, elderly, patients with hypovolemia, avoid abrupt withdrawal ADR: Serious: hypotension, hypertriglyceridemia, HTN, arrhythmia, bradycardia, Others: Inj site burning, stinging/pain, respiratory acidosis, apnea, fever, pancreatitis, discoloration of urine, nausea, vomiting, headache DDI: Serious Neuromuscular blockers causes bradycardia, Fentanyl enhances drug effect, Benzodiazepines increases drug effect, Lidocaine/Bupivacaine increases hypnotic effect of drug Monitor: Cardiac monitor,BP,oxygen saturation,arterial blood gas,metabolic acidosis,hyperkalemia,rhabdomyolysis or elevated CPK,hepatomegaly and progression of cardiac and renal failure,CNS function |
