Indications & Dose: ANGINA PO Adult ER Capsule: Initially 120-180mg OD 7-14 days (depends upon the formulation) max 480mg/day | PO Adult ER Tablet: 180mg OD 7-14 days (depends upon the formulation) max 360mg/day | HYPERTENSION PO Adult Modified-release capsule/ tablet: Initially 60-120mg BID, may be increased if needed max 360mg/day | SUPRAVENTRICULAR TACHYARRHYTHMIAS IV Adult Inj: Initially 15-20mg over 2 min, if response is inadequate, then 20-25mg after 15 min of initial dose, MD 5-15mg/h or adjust according to HR |
Contra: Hypersensitivity, sick sinus syndrome, second/third-degree AV block, severe hypotension, acute MI, pulmonary congestion
Precautions: Severe hepatic/renal impairment, HF, hypertrophic obstructive cardiomyopathy, left ventricular dysfunction ADR: Serious: AV block, bradycardia, hypotension, extrasystoles, palpitation, vasodilation, edema, Others: Rhinitis, pharyngitis, dyspnea, cough, weakness, myalgia, dizziness, nervousness, dyspepsia, constipation, vomiting, diarrhea DDI: Serious TCAs causes rises in imipramine levels by drug, Simvastatin/Lovastatin causes rises in statin levels by drug, Ranitidine/Cimetidine increases drug levels, Nitrates leads to additive hypotensive effects, Digoxin leads to risk of additive bradycardia, Phenytoin/Metoprolol/Propranolol/Tacrolimus levels increases, Cyclosporine/Carbamazepine/Quinidine levels increases & causes toxicity, Lithium toxicity raises Diet: With food Monitor: LFT, BP, ECG, HR |
