Indications & Dose: ANALGESIC PO Adult 30mg q4-6h, usual range 15-120mg q4-6h Child 0.5-1mg/kg/dose q4-6h, max 60mg/dose | IM/SC Adult 30mg q4-6h, prior opiate exposure patients require higher initial doses, usual range 15-120mg q4-6h Child 0.5-1mg/kg/dose q4-6h, max 60mg/dose | ANTI-TUSSIVE PO Adult 10-20mg/dose q4-6h, max 120mg/day Child 2-6 yr: 2.5-5mg q4-6h as needed max 30mg/day, 6-12 yr: 5-10mg q4-6h as needed max 60mg/dayfor nonproductive cough |
Contra: Hypersensitivity, comatose patients
Precautions: Severe renal/hepatic/pulmonary disease, adrenal insufficiency, head trauma, hypothyroidism, increased intracranial pressure, prostatic hypertrophy, undiagnosed abdominal pain, alcoholism, elderly, hypotension, shock, IBD/obstructive bowel disorders, myasthenia gravis, neonates ADR: Serious: Respiratory depression, bradycardia, peripheral vasodilation, blurred/double vision, flushing, Others: sweating, urinary urgency, nausea, vomiting, constipation, decreased gastric motility, urinary retention, Confusion, sedation, malaise, agitation, euphoria, headache, hallucinations, unusual dreams, apathy, mood changes, hypotension DDI: Serious Antimuscarinics causes significant impairment in tests of memory & attention in elder, Barbiturates increases CNS depressant effects of drug, Carbamazepine increases production of more potent metabolite of drug, Rifampicin markedly increases drug metabolism Diet: With/without food Monitor: Pain relief, respiratory & mental status, BP, HR |
