Indications & Dose: CYSTIC FIBROSIS IV Child 50mg/kg/dose q6-8h, max 8g/day | Inh Adult Nebulizer: 75mg TID (at least 4h apart) 4 wk & stop the treatment, do not repeat for 28 days after completion Child >7 yr: Same as adult dose | INFECT Mild-to-moderate IV/IM Child > 1 month: 30mg/kg TID | Moderate-to-severe IV/IM Child > 1 month: 30mg/kg q6-8h, max 120mg/kg/day (8g/day) | LIFE-THREATENING INFECT IV Adult 2g TID/QID, max 8g/day caused by P. aeruginosa | MENINGITIS IV Adult 2g q6-8h for Gram-negative Child <7 days: 30mg/kg over 3-5min BID, Neonate: 7-28 days: 30mg/kg over 3-5min TID/QID, 1 month-12 yr: 30mg/kg over 3-5min TID/QID, 12-18 yr: 1g TID or 2g BID Gram-negative | SYSTEMIC INFECT Moderate-to-severe IV Adult 1-2g BID/TID | IM Adult 2g q6-8h, max 8g/day for severe or life threatening infect | URINARY TRACT INFECT IV/IM Adult 0.5-1g q8-12h |
