Indications & Dose: ANALGESIA FOR PAINFUL PROCEDURES OF SHORT DURATION IM Adult 4mg/kg Child 4mg/kg | INDUCTION/MAINTENANCE OF ANESTHESIA IV Adult Induction: 1-4.5mg/kg over at least 1 min (2mg/kg usually produces 5-10min of anesthesia). Maintenance: 50-100% of induction dose as required Child Same as adult dose | IV Adult Induction: 0.5-2mg/kg (solution containing 1mg/ml) as inf Maintenance: 10-45µg/kg/min (using microdrip inf), rate adjusted according to response Child Same as adult dose | IM Adult Induction: 6.5-13mg/kg (10mg/kg usually produces 12-25min of anesthesia). Maintenance: 50-100% of induction dose as required Child Same as adult dose |
Contra: Hypersensitivity, history of CVA, psychotic disorders/patients prone to hallucinations, eye injury/increased IOP
Precautions: CVD, coronary artery disease, patients with CSF pressure elevation, chronic alcoholics/acutely alcohol-intoxicated, respiratory/renal/hepatic impairment ADR: Serious: Arrhythmia, bradycardia/tachycardia, hyper/hypotension, increased IOP, increased CSF pressure, anaphylaxis, respiratory depression, Others: laryngospasm, dependence with prolong use, hallucinations, irrational behavior, salivation increased, erythema, morbilliform rash, anorexia, nausea, vomiting, pain at Inj site, airway obstruction, apnea DDI: Serious Dextromethorphan increases adverse CNS-related drug reactions, Diazepam inhibits drug metabolism leads to reduces dose of drug required, Methylphenidate/Levothyroxine leads to marked HTN & tachycardia, Theophylline leads to seizures, Atracurium neuromuscular blockade is prolonged by drug, Morphine prolongs recovery time of drug Monitor: HR, BP, respiratory rate, transcutaneous O2 saturation, emergence reactions, cardiac function |
