Drug Pharmacology ::
I. Pharmacology. Folicacid is a B-complex vitamin that is essential for protein synthesis anderythropoiesis. In addition, the administration of folate to patientswith methanol poisoning may enhance the conversion of the toxicmetabolite formic acid to carbon dioxide and water, based on studies infolate-deficient primates. Note: Folic acid requiresmetabolic activation and is not effective for the treatment ofpoisoning by dihydrofolate reductase inhibitors (eg, methotrexate andtrimethoprim). Leucovorin (see Leucovorin Calcium) is the proper agentin these situations.

Indications. Adjunctive treatment for methanol poisoning.
Drug Contra-Indications ::
III. Contraindications. No known contraindications.
Drug Adverse Effects ::
IV. Adverse effects.
A. Rare allergic reactions have been reported after intravenous administration.
B. Use in pregnancy. FDA category A (see Table III–1). Folic acid is a recommended supplement.
Drug Lab Interactions ::
Drug or laboratory interactions. This agent may decrease hydantoin levels by enhancing their metabolism.
Drug Dose Management ::
Dosage and method of administration. Thedose required for methanol or ethylene glycol poisoning is notestablished, although 50 mg IV (children, 1 mg/kg) every 4 hours forsix doses has been recommended.
Drug Chemical Formulations ::
Formulations.
A. Parenteral. Sodium folate (Folvite), 5 mg/mL, 10-mL vials.
The suggested minimum stocking level to treat a 70-kg adult for the first 24 hours is six vials (5 mg/mL, 10 mL each).
