Indication & Dosage for Abacavir
- Oral HIV infection
- Adult: Combined with other antiretrovirals: 300 mg bid or 600 mg once daily.
- Child: Combined with other antiretrovirals: >3 mth: 8 mg/kg bid. Max: 600 mg daily. Renal impairment: Avoid in end stage renal disease. Hepatic impairment: Child pugh category A: 200 mg bid. Avoid in moderate-severe impairment.
Abacavir Administration
- May be taken with or without food.
- Contraindications
- Moderate to severe renal and hepatic impairment; end stage renal disease; history of hypersensitivity reactions. Pregnancy and lactation.
Special Precautions for Abacavir
- Contraindicated in:
- Hypersensitivity (rechallenge may be fatal).
- Lactation: Breastfeeding not recommended for HIV-infected patients.
- Use Cautiously in:
- Coronary heart disease.
- OB: Safety not established.
- Pedi: Safety not established in children <3 mo.
- Exercise Extreme Caution in:
- Patients positive for HLA-B*5701 allele (unless exceptional circumstances exist where benefits clearly outweigh the risks).
Drug Interactions with Abacavir —
- Drug-Drug:
- Alcohol increases blood levels.
- May increase methadone metabolism in some patients; slight increase in methadone dosing may be needed.
- Pregnancy Category (US FDA)
- Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available.
- Drugs should be given only if the potential benefit justifies the potential risk to the foetus.
Storage
- Oral: Store at 20-25°C.
- Mechanism of Action
- Abacavir is converted to the active form carbovir triphosphate intracellularly. It competitively inhibits the reverse transcriptase of retroviruses interfering with HIV viral RNA-dependent DNA polymerase resulting in inhibition of viral replication. Absorption: Rapidly absorbed after oral administration with an oral bioavailability of about 80%.
- Distribution: 50% bound to plasma proteins. Crosses the blood brain barrier.
- Metabolism: Undergoes intracellular metabolism to carbovir triphosphate which is the active metabolite.
- Excretion: Elimination half-life: About 1.5 hr after a single oral dose. Elimination by hepatic metabolism mainly by alcohol dehydrogenase and glucuronidation. The metabolites are excreted mainly in the urine.
Adv. Reactions/Side Effects of Abacavir —
- CNS: headache, insomnia.
- CV: myocardial infarction.
- GI: hepatotoxicity , diarrhea, nausea, vomiting, anorexia.
- Derm: rashes.
- F and E: lactic acidosis. Misc: hypersensitivity reactions.
Availability
- PO (Adults): 300 mg twice daily.
- PO (Children 3 mo–16 yr): 8 mg/kg twice daily (not to exceed 300 mg twice daily).
- Tablets: 300 mg
- Oral solution (strawberry/banana flavor): 20 mg/ml in 240-ml bottles.
- In combination with: lamivudine and zidovudine (Trizivir). See Appendix [not included in this PDA edition]
