Indications & Dose: CHOLECYSTITIS Mild-to-moderate IV Adult 1-2g TID 4-7 days | ENDOCARDITIS IV Adult 6g/day given in 3 equally divided doses 6 wks, may be given in combination with gentamicin Child 100mg/kg/day, upto 6g/day given in 3 or 4 equally divided doses 6 wks, may be given in combination with gentamicinfor native valve staphylococcal | IV Adult 6g/day given in 3 equally divided doses >6 wks, may be given in combination with gentamicin/rifampicin Child 100mg/kg/day, upto 6g/day given in 3 or 4 equally divided doses >6 wks, may be given in combination with gentamicin/rifampicinfor prosthetic valve staphylococcal | Prophylactic therapy IV/IM Adult 1g 30-60 mins before surgery Child 50mg/kg 30-60 mins before surgeryafter dental/upper respiratory tract procedures | INFECT Severe IV/IM Adult 1-1.5g q6h | Moderate-to-severe IV/IM Adult 0.5-1g q6-8h | PERINATAL GROUP B STREPTOCOCCAL INFECT IV Adult Initially 2g at the time of labor or rupture of membranes followed by 1g IV TID until deliveryintrapartum anti-infective prophylaxis | PERIOPERATIVE PROPHYLAXIS IV Adult 1-2g within 1h before surgery, may repeat in 2-5h intraoperatively, followed by 0.5-1g q6-8h for 24h post-operatively Child 20-30mg/kg 30-60 mins before surgery | PNEUMONIA IV/IM Adult 500mg BID | URINARY TRACT INFECT Uncomplicated IV/IM Adult 1g BID |
Contra: Hypersensitivity to cephalosporins
Precautions: sensitivity to ß-Lactam antibacterials, moderate renal impairment, false positive urinary glucose, false positive Coombs’ test, history of allergy, false Jaffé method of measuring creatinine level ADR: Serious: Antibiotic-associated colitis, arthritis, serum sickness-like reactions, neurotoxicity, renal impairment, blood disorders, allergic reactions, abnormal liver function tests, erythema multiforme, TEN, Others: vomiting, headache, dizziness, fever, Diarrhea, nausea, rash, electrolyte disturbances, cholestatic hepatitis, pain & inflammation at Inj site DDI: Serious Probenecid/Aminoglycosides significantly increases the risk of nephrotoxicity Monitor: Renal function test periodically when used in combination with other nephrotoxic drugs, LFT, CBC, signs of anaphylaxis (first dose) |
