Indications & Dose: BARTONELLA INFECT IV Adult 3mg/kg/day with other anti-infectives. Gentamicin given for first 2 wks & other anti-infectives continued for 6 wks | ENDOCARDITIS IV/IM Adult 3mg/kg/day in 3 equally divided doses with other antibiotics Child Same as adult dosenative or prosthetic valve | GRANULOMA INGUINALE IV Adult 1mg/kg TIDDonovanosis | MENINGITIS IV Child 0-7 days: <2 kg: 2.5mg/kg q18-24h, >2 kg: 2.5mg/kg BID, 8-28 days: <2 kg: 2.5mg/kg BID/TID, >2 kg: 2.5mg/kg TID | IV/IM Adult Enterococcus sp/P. aeruginosa: Loading dose 2mg/kg, then 1.7mg/kg/dose TID (along with bactericidal), Listeria: 5-7mg/kg/day (with penicillin) 1 wk Child Same as adult dose | OPHTHALMIC INFECT DUE TO SUSCEPTIBLE BACTERIA TP Adult Ointment: Instill ½ inch 2-3 times/day q3-4h, Solution: Instill 1-2 drops q2-4h, may increase to 2 drops/h for severe infect | PELVIC INFLAMMATORY DISEASE IM Adult Initially 2mg/kg then 1.5 mg/kg TID in conjunction with clindamycin (900mg IV TID) | PERIOPERATIVE PROPHYLAXIS IV Adult Head & neck surgery: 1.5mg/kg with clindamycin immediately prior to surgery, may be repeated if surgery is prolonged/ major blood loss occurs | PREVENTION OF ENDOCARDITIS IV/IM Adult 1.5mg/kg with ampicillin/ amoxicillin Child Same as adult dose | PSEUDOMONAL LUNG INFECTION IN CYSTIC FIBROSIS IV Child Inj/Inf:1 month–18 yr: 3mg/kg over 3 min TID | SUPERFICIAL INFECT TP General 0.1% cream/ointment: Gently apply small amount to the cleansed affected area TID/QIDdry, psoriatic, eczematous | TULAREMIA IV/IM Adult 5mg/kg/day 10 days Child 5-7.5mg/kg/day in 3 divided doses 10 days | URINARY TRACT INFECT Uncomplicated IM Adult 160mg OD or 5mg/kg single dose Child 5mg/kg single dose, max 300mg

Contra: Hypersensitivity, perforated tympanic membrane

Precautions: Avoid prolonged use, myasthenia gravis, parkinsonism, pre-existing renal insufficiency, vestibular impairment, hypocalcemia

ADR: Serious: Neuromuscular blockade, neurotoxicity, nephrotoxicity, ototoxicity, hepatotoxicity, hepatic necrosis, hypersensitivity reactions, conjunctival edema, severe retinal ischemia, Others: dizziness, nystagmus, visual disturbances, photophobia, increased salivation, exfoliative dermatitis, urticaria, alopecia, hyperemia

DDI: Serious Amphotericin-B decreases drug clearance, Neuromuscular blockers increases & prolongs neuromuscular blockade & leads to fatal respiratory depression, Cephalosporins increases nephrotoxic effects, Cisplatin increases renal toxicity, Ciclosporin increases risk of nephrotoxicity, Clindamycin leads to acute renal failure, Vit K/Sedatives/hypnotics leads to hypoprothrombinemia, Digoxin levels increase by drug

Diet:

Monitor: Urinalysis, urine output, BUN, SeCr, hearing should be test before, during, and after treatment
