Indications & Dose: CARDIAC ALLOGRAFT REJECTION PO/IV Adult 1.5 BID (as mofetil) 3g total daily dosage) | HEPATIC ALLOGRAFT REJECTION PO Adult 1.5g BID (as mofetil) | IV Adult 1g BID (as mofetil) | RENAL ALLOGRAFT REJECTION PO Adult Delayed-release tablet: 720mg BID Child 3 month-18 yr: Suspension (as mofetil) as 600mg/m2 BID, max 1g BID, for children with BSA 1.25-1.5m2: 750mg BID (capsule), >1.5m2: 1g BID (capsule/tablet) or 5-16 yr (as sodium): 400mg/m2 BID, max 720mg BID, for children with BSA 1.19-1.58m2: 1080mg/day (three 180mg tablets BID), >1.58m2: 1.44g/day (four 180mg tablets BID) | IV Adult 1g BID (as mofetil) |
Contra: Hypersensitivity, avoid IV use to patients who are allergic to polysorbate 80
Precautions: Renal impairment, PUD, patients with deficiency of hypoxanthine-guanine phosphoribosyltransferase, Use of IUCD, avoid exposure to sunlight/UV ADR: Serious: Renal impairment, pancreatitis, interstitial lung disorders, hepatitis, fatal pulmonary fibrosis, renal tubular necrosis, conjunctivitis, blurred vision, GI hemorrhage, hematuria, impotence, blood disorders, hypersensitivity reactions, angioedema, anaphylaxis, increased risk of infect & certain malignancies, hypotension/HTN, peripheral edema, arthralgia, dyspnea, disturbances of electrolytes & blood lipids, tachycardia, Others: pleural effusion, cough, rash, alopecia, acne, dizziness, insomnia/somnolence, paresthesia, agitation, depression, anxiety, facial abnormalities, tremor, GI disturbances, diarrhea, vomiting, fever, pain, headache, asthenia, hyperglycemia DDI: Serious Azathioprine causes bone marrow suppression, Valaciclovir/Ganciclovir/Valganciclovir causes neutropenia, Tacrolimus/Probenecid increases drug levels, Vaccines (Live) leads to risk of generalized infect, Colestyramine/Aluminium hydroxide/Magnesium Hydroxide reduces drug absorption, Antibacterials reduces drug bioavailability, Corticosteroids/Ciclosporin/Lansoprazole/Rifampicin reduces drug levels Diet: On empty stomach Monitor: Complete blood count, renal function, LFTs, signs & symptoms of infect |
