Indications & Dose: DYSPEPSIA PO Adult 1-5g in water as required | METABOLIC ACIDOSIS PO Adult 57mmol (4.8g) or increase if needed | Severe IV Adult Continuous infusion 1.26% (150 mmol/L) solution or by slow IV injection of stronger (hypertonic) solution of up to 8.4% (1000mmol/L) | SEVERE HYPERKALEMIA WITH ACIDOSIS Severe PO Adult 50-100mL of 8.4% solution | URINE ALKALINAZATION PO Adult Initially 48mEq (4g), then 12-24mEq (1-2g) q4h, titrate dose based on urinary pH max 16g/day or 200mEq (>60 yr of age max 8g or 100mEq) Child 1-10mEq (84-840mg)/kg/day in divided doses q4-6h titrate dose based on urinary pH |
Contra: alkalosis, hypernatremia, hypocalcemia, severe pulmonary edema, hypokalemia, seizures, vomiting, acute ingestion of mineral acids
Precautions: HF, edema, cirrhosis, renal failure ADR: Serious: cerebral hemorrhage, CHF, edema, hypokalemia, cardiac arrest, paralytic ileus, hyperosmolarity, cyanosis, apnea, tetany, Others: belching, hypernatremia, alkalosis, hypocalcemia DDI: Serious Pseudoephedrine/Ephedrine causes tremors, anxiety, insomnia, tachycardia, Tetracyclines efficacy is reduced/abolished, Methenamine raises urinary pH >5.5, Flecainide reduces drug excretion, Aspirin salicylates levels are reduced Diet: 1-3h after food |
