Indications & Dose: ENDOCARDITIS Prophylactic therapy IV Adult 400mg single dose with gentamicin before before the procedureIn high-risk patients undergoing dental or other procedures | ORTHOPAEDIC SURGERY Prophylactic therapy IV Adult 400mg single dose with gentamicin before anesthesiaIn high-risk patients undergoing dental or other procedures | SERIOUS BACTERIAL INFECT IV Adult Inf: Initially 6mg/kg/day over 30 min, followed by 3mg/kg on subsequent day. In more severe cases 6mg/kg q12h for first 3doses, followed by 6mg/kg/day Child Neonate: Initially 16mg/kg single dose 1 day, then 8 mg/kg OD, 1 month–18 yr: 10mg/kg q12h 3 doses, then 6mg/kg OD, max 400mg. In more severe cases 10mg/kg q12h for 3 doses then 10mg/kg ODGram-positive organisms

Precautions: Pre-existing renal impairment

ADR: Serious: SJS, angioedema, bronchospasm, red-man syndrome, TEN, Others: GI disturbances, headache, dizziness, thrombophlebitis, blood disorders, fever, skin rash, pruritus, erythema, rigors, exfoliative dermatitis, erythema multiforme

Monitor: LFTs,renal function test
