Indications & Dose: ANTIBIOTIC-ASSOCIATED COLITIS PO Adult 25, 000U QID 7-10 days | EMPYEMA IM Child Infants: <2.5 kg: 900U/kg/day in 2-3 divided doses, >2.5 kg: 1000U/kg/day in 2-3 divided doses | STAPHYLOCOCCAL PNEUMONIA IM Child Infants: <2.5 kg: 900U/kg/day in 2-3 divided doses, >2.5 kg: 1000U/kg/day in 2-3 divided doses | SUSCEPTIBLE BACTERIAL INFECT Acute TP Adult Ophthalmic (500U/g): Instill 1/4” to 1/2” ribbon q3-4h into conjunctival sac Child Same as adult dose | Mild-to-moderate TP Adult Ophthalmic (500U/g): Instill 1/4” to 1/2” ribbon 2-3 times/day into conjunctival sac 7-10 days Child Same as adult dose |
Precautions: withdraw bacitracin if renal toxicity occurs, avoid concomitant therapy with nephrotoxic drugs
ADR: Serious: tubular & glomerular necrosis, anaphylaxis, Others: nausea, vomiting, pain at Inj site, rashes DDI: Serious Vancomycin increases risk of nephrotoxicity Monitor: Fluid intake & urinary output |
