Indications & Dose: DIABETIC KETOACIDOSIS Mild-to-moderate IV/IM/SC Adult > 20 yr: 0.2U/kg as IV bolus followed by 0.2 u/kg as IM/SC, Intermittent: 0.1U/kg as IM/SC, <20 yr: IM/SC Initially 0.1-0.3U/kg as a bolus followed by 0.1U/kg every h. Continue until acidosis clears and then decrease to 0.05U/kg/h until SC replacement dosing is initiated Child Same as <20 yr adult dose | TYPE 1/SEVERE TYPE 2 DIABETES MELLITUS Combination therapy SC Adult Dose is individualized based on results of blood glucose with insulin lispro protamine |
