Indications & Dose: ANAPLASTIC ASTROCYTOMA PO/IV Adult Initially 150mg/m2/day 5 days, repeat q28days. Subsequent doses 100-200mg/m2/day 5 days per treatment cycle based on hematologic toleranceRefractory | MALIGNANT MELANOMA PO Adult 200mg/m2/day 5 days q28days | NEWLY DIAGNOSED GLIOBLASTOMA MULTIFORME PO Adult Concomitant phase: Initially 75mg/m2/day 42 days with focal radiotherapy. Continue the therapy or withhold based on toxicity. After 4 wk of completing concomitant phase temozolomide is continued as monotherapy, 150mg/m2 OD 5 days of 28day cycle. In cycle 2 dose is increased to 200mg/m2 5 days | RECURRENT OR PROGRESSIVE MALIGNANT GLIOMAS PO Adult 200mg/m2/day 5 days, repeated q28days, in patients who have received previous courses of chemotherapy the dose should be reduced to 150mg/m2 for first cycle of therapy, may increase to 200mg/m2 for subsequent courses if no hematological toxicity Child >3 yr: Same as adult dosePreviously untreated with chemotherapy

Precautions: Elderly, severe hepatic/renal impairment, active infect, decreased bone marrow reserve, chronic debilitating illness

ADR: Serious: seizure, hemiparesis, peripheral edema, blood disorders, viral infect, tinnitus, Others: amnesia, insomnia, incontinence, blurred vision, pharyngitis, weakness, fatigue, headache, fever, dizziness, alopecia, rash, nausea, vomiting, constipation, anorexia

DDI: Serious Valproic acid modestly reduces drug clearance

Diet: With/without food

Monitor: CBC with differential & platelets
