Benztropine
Drug Pharmacology ::
I. Pharmacology. Benztropineis an antimuscarinic agent with pharmacologic activity similar to thatof atropine. The drug also exhibits antihistaminic properties.Benztropine is used for the treatment of parkinsonism and the controlof extrapyramidal side effects associated with neuroleptic use.
Drug Indications ::
Indications. Benztropineis an alternative in adults to diphenhydramine (the drug of choice forchildren) for the treatment of acute dystonic reactions associated withneuroleptics or metoclopramide. It has a longer duration of action thandoes diphenhydramine and is administered twice daily. Note: It is not effective for tardive dyskinesia or neuroleptic malignant syndrome (see Seizures).

Angle-closure glaucoma.
Obstructive uropathy (prostatic hypertrophy).
Myasthenia gravis.
Notrecommended for children under 3 years by the manufacturer;alternatively, use diphenhydramine (see Diphenhydramine) or considerbenztropine if the patient is unresponsive or hypersensitive todiphenhydramine and is experiencing a severe or life-threateningsituation (eg, dystonic laryngeal or pharyngeal spasms).

Drug Adverse Effects ::
IV. Adverse effects
Adverseeffects include sedation, blurred vision, tachycardia, urinaryhesitancy or retention, and dry mouth. Adverse effects are minimalafter single doses.
Use in pregnancy. FDAcategory C (indeterminate). However, this does not preclude its acute,short-term use for a seriously symptomatic patient (see Table III–1).
Drug Lab Interactions ::
Drug or laboratory interactions
Benztropinehas additive effects with other drugs that exhibit antimuscarinicproperties (eg, antihistamines, phenothiazines, cyclic antidepressants,and disopyramide).
Slowing of GI motility may delay or inhibit absorption of certain drugs.
Drug Dose Management ::
Dosage and method of administration
Parenteral. Give 1–2 mg IV or IM (children 3 years old, 0.02 mg/kg and 1 mg maximum). May repeat dose in 15 minutes if unresponsive.
Oral. Give1–2 mg PO every 12 hours (children 3 years old, 0.02 mg/kg and 1 mgmaximum) for 2–3 days to prevent recurrence of symptoms. Maximumrecommended dose for adults is 6 mg/day.
Drug Chemical Formulations ::
Formulations
Parenteral. Benztropine mesylate (Cogentin), 1 mg/mL, 2-mL ampules.
Oral. Benztropine mesylate (Cogentin, generic), 0.5-, 1-, and 2-mg tablets.
The suggested minimum stocking level to treat a 70-kg adult for the first 24 hours is 6 mg (three ampules, 2 mL each).
