Indications for Drugs ::
Severe Essential Hypertension, Hypertensive Crisis,
Drug Dose ::
Severe Essential Hypertension 20-40 mg IM/IV; repeat as necessary Hypertensive Crisis 20-40 mg IV/IM; repeat PRN Pregnancy-associated 5-10 mg IV/IM initially, THEN 5-10 mg q20-30min PRN, OR 0.5-10 mg/hr IV infusion
Contraindication ::
Hypersensitivity to hydralazine, Coronary artery disease, Mitral valve rheumatic heart disease.
Drug Precautions ::
May induce SLE-type syndrome (usually at >200 mg/day); instruct patients to report joint/chest pain or fever; consider discontinuation if occurs Use caution in CVA, severe renal impairment, volume depletion, preexisting hypotension, concurrency with other hypotensive agents, CAD (potential contraindication) Discontinue slowly to avoid rapid rise in blood pressure Increases fluid and sodium retention; may require treatment or increase in diutretic dose Peripheral neuritis reported; treat symptoms with pyridoxine
Drug Side Effects ::
Common Headache, anorexia, nausea, vomiting, diarrhea, palpitations, tachycardia, angina pectoris. Less Frequent Digestive: constipation, paralytic ileus. Cardiovascular: hypotension, paradoxical pressor response, edema. Respiratory: dyspnea. Neurologic: peripheral neuritis evidenced by paresthesia, numbness, and tingling, dizziness: tremors; muscle cramps; psychotic reactions characterized by depression, disorientation, or anxiety. Genitourinary: difficulty in urination. Hematologic: blood dyscrasias, consisting of reduction in hemoglobin and red cell count, leukopenia, agranulocytosis, purpura, lymphadenopathy; splenomegaly. Hypersensitive Reactions: rash, urticaria, pruritus, fever, chills, arthralgia, eosinophilia, and, rarely, hepatitis. Other: nasal congestion, flushing, lacrimation, conjunctivitis.

Direct vasodilator; dilates arterioles with little effect on vein; decreases systemic resistance, which subsequently decreases blood pressure.

Hypotensives, diazoxide, NSAIDs, oestrogens, sympathomimetics.
