Indications & Dose: ANGINA PECTORIS PO Adult Initially 12.5mg BID, increased after 2 days to 25mg BID | HEART FAILURE PO Adult IR tablet: 3.125mg BID 2 wk, may increase to 6.25mg BID if tolerated. Double the dose q2wk to the highest dose tolerated. In mild-to-moderate HF max dose 25mg BID (<85 kg), 50mg BID (>85 kg), severe HF 25mg BID. Prior to initiating therapy, other medications should be stabilized & fluid retention minimized. ER tablet: Initially 10mg/day 2 wk, if dose is tolerated, increase to 20mg, 40mg, 80mg over successive intervals of at least 2 wk. Maintain on lower dose if higher dose is not tolerated | HYPERTENSION PO Adult IR tablet: 6.25mg BID, if tolerated maintain for 1-2 wk, then increase to 12.5mg BID. If needed further increase to max 25mg BID after 1-2 wk. ER: Initially 20mg OD, if tolerated maintain for 1-2 wk then increase to 40mg OD if necessary max 80mg OD | LEFT VENTRICULAR DYSFUNCTION AFTER MI PO Adult IR tablet: 3.125-6.25mg BID, gradually increase dose at intervals of 3-10 days as tolerated. Target dose 25mg BID, ER tablet: Initially 10-20mg/day gradually increase dose at intervals of 3-10 days as tolerated. Target dose 80mg/day |
| Contra: hypersensitivity, bronchospastic conditions, second/third degree AV block, severe chronic HF, sick sinus syndrome, cardiogenic shock, severe hepatic impairment, severe bradycardia
Precautions: Renal impairment, pulmonary disease, DM, hypoglycemia, thyrotoxicosis, peripheral vascular disease, hypotension, respiratory depression, elderly ADR: Serious: Bradycardia, HF, AV block, renal dysfunction, hypoglycemia, bronchospasm, pulmonary edema, thrombocytopenia, anaphylaxis, angina pectoris, orthostatic hypotension, blurred/abnormal vision, hematuria, hyperuricemia, arthralgia, hyponatremia, glycosuria, gout, Others: URTI, dizziness, fatigue, anxiety, depression, insomnia, nightmares, dry eyes, stuffy nose, sinusitis, pharyngitis, nausea, diarrhea, constipation, decreased libido, erectile dysfunction, hyperglycemia, muscle cramps, wheezing, memory loss, dyspnea, pruritus, rash, weight gain, lupuslike syndrome, viral infect DDI: Serious Digoxin bioavailability increases, Rifampicin increases drug clearance, Fluoxetine increases drug level, Ciclosporin levels increases Diet: With food Monitor: HR,BP, BUN, LFT |
