Indications & Dose: VENTRICULAR ARRHYTHMIAS PO Adult Loading dose 400mg then 200-250mg TID/QID given 2h after loading dose, MD 600-900mg/day in divided doses, max 1.2g/day | IV Adult Inf: 100-250mg (25mg/min) then 250mg over 1h & 250mg next 2h, MD 500µg/min. When appropriate switch to PO (200-250mg TID/QID) |
Contra: Hypersensitivity, cardiogenic shock, second/third-degree AV block
Precautions: Sinus node dysfunction, conduction disorders, bradycardia, hypotension, HF, hepatic impairment ADR: Serious: Hepatic necrosis, blood disorders, heart block & AV dissociation, atrial fibrillation, SJS, blurred vision, tinnitus, Others: nausea, vomiting, dyspnea, skin rashes, Dizziness, nervousness, confusion, fatigue, sleep disorder, tremor, paresthesia, edema DDI: Serious Rifampicin increases drug clearance, Theophylline levels are increases and causes toxicity by drug, Fluvoxamine markedly increases the AUC of drug, Ritonavir/Propafenone raises drug levels, Opioids reduces drug absorption & follows MI, Phenytoin reduces drug levels, Sparfloxacin reduces QT prolongation Diet: With food Monitor: Cardiac status, CNS changes |
