Indications & Dose: HEMORRHOIDS TP Adult 0.25% of cream/gel/ointment/suppository alone or in combination with other anorectal agents applied HS or in morning & after bowel movements QID Child >12 yr: Same as adult dose | HYPOTENSION IV Adult Bolus: 0.1-0.5mg/dose q10-15min as needed (initial dose should not exceed 0.5mg), Inf: Initially 100-180 µg/min or 0.5µg/kg/min, titrate to desired response Child Bolus: 5-20µg/kg/dose q10-15min as needed, Inf: 0.1-0.5µg/kg/min | MYDRIASIS TP Adult 1-2 drops of solution (2.5/10%) applied on upper limbus, may be repeated q10-60min if needed | NASAL DECONGESTION PO Adult 10-20mg q4h as needed 1 wk Child 4-6 yrs: 2.5mg q4h as needed <7 days; 6-12 yr: 5mg q4h as needed for < 1 wk | PAROXYSMAL SUPRAVENTRICULAR TACHYCARDIA IV Adult 0.25-0.5mg/dose over 20-30sec Child 5-10µg/kg/dose over 20-30sec | SHOCK IV Adult Bolus: 0.1-0.5mg/dose q10-15min as needed (initial dose should not exceed 0.5mg), Inf: Initially 100-180 µg/min or 0.5µg/kg/min, titrate to desired response Child Bolus: 5-20µg/kg/dose q10-15min as needed, Inf: 0.1-0.5µg/kg/min |
Contra: Hypersensitivity, HTN, ventricular tachycardia, use with/within 14 days of MAOI therapy
Precautions: Sulfite sensitivity, asthma, bowel obstruction/narrowing, IOP, prostatic hyperplasia, myocardial disease, severe CAD, hyperthyroidism, partial heart block, bradycardia, arteriosclerosis, unstable vasomotor syndrome, type 1 DM, elderly ADR: Serious: Arrhythmias, blood disorders, tremor, blurred vision, tachycardia, bradycardia, HTN, Others: palpitations, rebound miosis, rebound hyperemia, photophobia, eye burning/stinging, decreased IOP, sneezing, local irritation, nausea, vomiting, gastric irritation, anorexia, rash, drowsiness, insomnia, hallucinations, nervousness, prolonged psychosis, orofacial dystonia, asthmatic episodes, headache, weakness, anxiety DDI: Serious GA causes cyanosis, bradycardia & HTN with eye drops of drug, Atropine causes HTN & other serious adverse effects of IV & eye drops of drug, MAOIs causes potentially life-threatening hypertensive crisis, Guanethidine/Clonidine increases BP by drug, Imipramine increases pressor effects of IV inf of drug Monitor: BP/mean arterial pressure, pulse, cardiac output, pulmonary capillary wedge pressure, monitor infusion site closely |
