LEVOSULPIRIDE
D2 RECEPTOR BLOCKER | ANTI-PSYCHOTIC
PK: A: Slowly absorbed E: Urine, feces

Indications & Dose: SCHIZOPHRENIA PO Adult Initially 200-400mg BID max 1.2g BID (in patients with positive symptoms) or 800mg/day (in patients with negative symptoms). Patients with mixed positive & negative symptoms: 400-600mg BID Child >14 yr: Same as adult dose Elderly Initiate with lower dose & adjust as required | IM Adult 200-800mg/day

Contra: Pre-existing CNS depression/coma, bone-marrow suppression, pheochromocytoma, prolactin-dependent tumours, undue exposure to direct sunlight

Precautions: Manic/hypomanic patients, impaired hepatic/renal, cerebrovascular, respiratory function, those with angle-closure glaucoma, history of jaundice, Parkinsonism, DM, hypothyroidism, myasthenia gravis, paralytic ileus, prostatic hyperplasia, urinary retention (if possible avoid), epilepsy/history of seizures, debilitated patients, elderly, should not drive/operate machinery, presence of acute infect/leucopenia, avoid abrupt withdrawal

ADR: Serious: Blurred vision, mydriasis, tachycardia, ECG changes, orthostatic hypotension, priapism, impotence, hypersensitivity reactions, jaundice, blood disorders, extrapyramidal dysfunction, hyperprolactinemia, EEG changes & convulsions, cardiac arrhythmias/aspiration, asphyxia, Others: nasal congestion, inhibition of ejaculation, Sleep disturbances, overstimulation, agitation, dry mouth, constipation, difficulty with micturition, weight gain, hyperglycemia & altered glucose tolerance, delirium, insomnia, drowsiness, depression

DDI: Serious Moclobemide may enhance the adverse effects of drug, Magnesium Hydroxide/Aluminium hydroxide/Sucralfate reduces drug absorption

Monitor: Eye examination, CBC (if patient develop infect/fever)
