Indications & Dose: COMMUNITY-ACQUIRED PNEUMONIA PO/IV Adult 600mg BID 10-14 days Child 7 days-11 yr: 10mg/kg TID, >12 yr: Same as adult dose | NOSOCOMIAL PNEUMONIA PO/IV Adult 600mg BID 10-14 days Child 7 days-11 yr: 10mg/kg TID, >12 yr: Same as adult dose | SKIN/SKIN STRUCTURE INFECT Uncomplicated PO Adult 400mg BID 14-28 days Child <5 yr: 10mg/kg TID, 5-11 yr: 10mg/kg BID, >12 yr: 600mg BID 14-28 days | Complicated PO/IV Adult 600mg BID 10-14 days Child 7 days-11 yr: 10mg/kg TID, >12 yr: Same as adult dose includes diabetic infect | VANCOMYCIN-RESISTANT ENTEROCOCCUS FAECIUM INFECT PO/IV Adult 600mg BID 14-28 days Child 7 days-11 yr: 10mg/kg TID, >12 yr: Same as adult dose

Contra: Hypersensitivity, concurrent use/within 2 wks of MAO inhibitors

Precautions: Carcinoid syndrome, uncontrolled HTN, hyperthyroidism, pheochromocytoma, history of seizures, bipolar depression, schizophrenia/acute confusional states, renal/hepatic impairment, sympathomimetics, vasopressors, dopaminergic agents, SSRIs, TCAs, serotonin 5-HT1B, 1D receptor agonists, meperidine

ADR: Serious: Convulsions, increased ALT, AST, alkaline phosphatase, increased BUN, blood disorders, pancreatitis, blurred vision, tinnitus, polyuria, reversible myelosuppression, Others: diarrhea, headache, rash, nausea, vomiting, constipation, taste alteration, vaginal moniliasis, fungal infect, insomnia, dizziness, fever

DDI: Serious MAOIs enhance the drug adverse effects, Duloxetine/Amitriptyline/Mirtazapine/Paroxetine/SSRIs/Venlafaxine leads to serotonin syndrome, Rifampicin reduces drug levels

Diet: With/without food

Monitor: CBC, coagulation studies, bleeding,previous or concomitant antibiotic therapy in chronic infect patients, visual function with extended therapy
