Drugs Class ::

 Action Competitively antagonizes histamine at H1 receptor sites.

Indications for Drugs ::

 Indications Relief of nasal and non-nasal symptoms of seasonal and perennial allergic rhinitis.

Drug Dose ::

 Route/Dosage

Histex CT Tablets Adults and children (12 yr and over): PO 8 mg q 12 hr.

Children (6 to 12 yr): PO 4 mg q 12 hr.

Histex Pd Liquids Adults and children (6 yr and over): PO 5 mL qid.

Children (18 mo to 6 yr): PO 2.5 mL qid.

Children (9 to 18 mo): PO 1.25 to 2.5 mL qid.

Pediatex Adults and children (6 yr and over): PO 10 mL qid.

Children (18 mo to 6 yr): PO 5 mL qid.

Children (9 to 18 mo): PO 3.75 to 5 mL qid.

Children (6 to 9 mo): PO 3.75 mL qid.

Children (3 to 6 mo): PO 2.5 mL qid.

Children (1 to 3 mo): PO 1.25 mL qid.

Contraindication ::

 Contraindications MAO inhibitor therapy; patients with narrow-angle glaucoma, urinary retention, or breathing problems such as emphysema or chronic bronchitis; hypersensitivity to any component of the product.

Drug Side Effects ::

 Adverse Reactions

CNS: Sedation; dizziness; headache; nervousness; nausea; anorexia; heartburn; weakness; excitability in children.EENT: Diplopia. GI: Vomiting; diarrhea; dry mouth. GU: Polyuria.

Drug Interactions ::

 Interactions

Alcohol, barbiturates (eg, phenobarbital), tricyclic antidepressants (eg, amitriptyline), other CNS depressants: Effects may be enhanced by carbinoxamine.

MAO inhibitors: May prolong and intensify the effects of carbinoxamine.

Drug Assesment ::

 Assessment/Interventions

  • Obtain patient history, including drug history, and any known allergies. Note history of narrow angle glaucoma, stenosing peptic ulcer, pyloric obstruction, prostatic hypertrophy, bladder neck obstruction, asthma, emphysema, chronic bronchitis, and concurrent or recent use of MAO inhibitors or oral anticoagulants.
  • Ensure that orders for liquid carbinoxamine contain the concentration (ie, 1.75 mg/5 mL or 4 mg/5 mL) of the liquid to be administered.
  • Assess for allergy symptoms (eg, rhinitis, nasal congestion, sneezing, itching, watery eyes) before and periodically throughout therapy.
  • Monitor patient for dizziness and excessive drowsiness. If noted hold therapy and notify health care provider.
  • Monitor patient for CNS, GI, and general body side effects. Inform health care provider if noted and significant.
OVERDOSAGE: SIGNS & SYMPTOMS
  Young children predominant symptoms include: excitation, hallucination, ataxia, incoordination, tremors, flushed face, fever, convulsions, fixed and dilated pupils, coma, death; adults: fever, flushing, excitement, convulsions, postictal depression preceded by drowsiness and coma, respiratory depression

Drug Storage/Management ::

 Administration/Storage

  • Give ½ or 1 tablet q 12 hr as prescribed without regard to meals. Give with food if GI upset occurs.
  • Tablet can be broken in half without affecting release of medication. Tablets or half-tablets should not be crushed or chewed.
  • Give liquid dose 4 times/day as prescribed without regard to meals. Give with food if GI upset occurs.
  • Use caution when administering liquid. Note that there are 2 concentrations (1.75 mg/5 mL and 4 mg/5 mL) available. Administering the wrong liquid can result in an overdose or underdose.
  • Use dosing spoon or syringe to measure liquid dose.
  • Store liquid at controlled room temperature (59° to 86°F).

Drug Notes ::

 Patient/Family Education

  • Explain name, dose, action, and potential side effects of drug.
  • Advise patient to take ½ or 1 tablet q 12 hr as prescribed without regard to meals, but to take with food if GI upset occurs.
  • Advise patient, parent, or caregiver that tablets or half-tablets should not be crushed or chewed but should be swallowed whole.
  • Advise patient, parent, or caregiver that liquid medication is taken 4 times/day as prescribed.
  • Advise patient, parent, or caregiver to use dosing spoon or syringe to measure liquid dose.
  • Advise patient, parent, or caregiver that if allergy symptoms are not controlled not to increase the dose of medication or frequency of use but to inform their health care provider. Inform patient that larger or more frequent dosing does not increase effectiveness and may cause drowsiness.
  • Advise patient that medication may cause drowsiness or dizziness and to not drive or perform other activities requiring mental alertness until tolerance is determined.
  • Caution patient that alcohol and other CNS depressants (eg, sedatives) will have additional sedative effects if taken with carbinoxamine.
  • Caution patient not to take any OTC antihistamines while taking this medication unless advised by their health care provider.
  • Advise patient to take sips of water, suck on ice chips or sugarless hard candy, or chew sugarless gum if dry mouth occurs.
  • If patient is to have allergy skin testing, advise to not take the medication for at least 7 days before the skin testing.
  • Advise women to notify health care provider if pregnant, plan on becoming pregnant, or breastfeeding.
  • Instruct patient to stop taking drug and immediately report any of these symptoms to health care provider: dizziness, excessive drowsiness.
  • Caution patient to not take any prescription or OTC medications, or dietary supplements unless advised to do so by health care provider.
