Indications & Dose: FEBRILE NEUTROPENIA Empirical therapy IV Adult 2g TID 1 wk or until neutropenia resolves Child <40 kg: 50mg/kg TID | INFECT Severe IV/IM Child >2 months: 150mg/kg/day in 3 equally divided doses | Mild-to-moderate IV/IM Child >2 months: 100-150mg/kg/day in 3 equally divided doses | INTRA-ABDOMINAL INFECT Complicated IV Adult 2g BID 7-10 days in combination with metronidazole | PNEUMONIA Mild-to-moderate IV Adult 1-2g BID 10 days Child <40 kg:50mg/kg BID | SKIN/SKIN STRUCTURE INFECT Uncomplicated IV Child <40 kg: 50mg/kg BID | Moderate-to-severe/Uncomplicated IV Adult 2g BID 10 days | URINARY TRACT INFECT IV Child <40 kg: 50mg/kg BID associated with pyelonephritis | Severe IV Adult 2g BID 10 days associated with pyelonephritis and/or with concurrent bacteremia | Mild-to-moderate IV/IM Adult 0.5-1g BID 7-10 days associated with pyelonephritis and/or with concurrent bacteremia |
Contra: Hypersensitivity to cephalosporins
Precautions: History of allergy, penicillin-sensitive patients, false positive Coombs’ test, renal impairment, false Jaffé method of measuring creatinine level, false-positive reactions for glucose using copper-reduction reactions ADR: Serious: Blood disorders, hypersensitivity reactions, bleeding complications, hypoprothrombinemia, pseudomembranous colitis, CNS toxicity, convulsions, cholestatic jaundice, hepatitis, Others: Nausea, vomiting, diarrhea, pain at Inj site, thrombophlebitis DDI: Serious Probenecid increases levels/effects of drug, Typhoid vaccine/BCG levels/effects decreases by drug Monitor: Obtain specimen for culture & susceptibility before first dose, signs of anaphylaxis (first dose) |
