Indications & Dose: HEART FAILURE PO Adult Initially 2mg in the morning MD 4mg/day (as erbumine) or 2.5mg MD 5mg/day (as arginine) | HYPERTENSION PO Adult Initially 4mg/day, increase dose based on response, usual range 4-8mg/day in 2 divided doses, increase at 1-2 wk interval, max 16mg/day Elderly >65 yr: Initially 4mg/day, MD 8mg/day | Combination therapy PO Adult Patients receiving diuretics should discontinue diuretic prior to initiation of perindopril. If not initiate low dose of perindopril (2-4mg/day) for first 2 wk, adjust the dose of diuretic/perindopril based on response | ISCHEMIC HEART DISEASE PO Adult Initially 4mg/day 2 wk MD 8mg/day (as erbumine) or 5mg/day 2 wk MD 10mg/day (arginine) Elderly Initially 2 (erbumine) or 2.5mg (arginine) OD for the first wk | STABLE CORONARY ARTERY DISEASE PO Elderly >70 yr: Initially 2mg/day 1 wk, increase to 4mg/day 1 wk if tolerated, then increase as tolerated to 8mg/day |
Contra: Hypersensitivity, history of angioedema with ACE inhibitor, aortic stenosis/outflow tract obstruction
Precautions: Hepatic failure, renal impairment, renal artery stenosis, hyperkalemia, cough, black patients with HTN, history of idiopathic/hereditary angioedema, renovascular disease/suspected renovascular disease, HF, patients who are likely to be sodium/water depleted ADR: Serious: Hyperkalemia, angioedema, tachycardia, edema, Others: rhinitis, pharyngitis, nausea, vomiting, diarrhea, abdominal pain, proteinuria, UTI, erectile/other male sexual dysfunction, decreased libido, menstrual disorder, arm, back, joint pain, leg, neck, hypertonia, myalgia, arthritis, cough, fever, rash, dizziness, fatigue, headache, insomnia, sleep disorder, weakness, asthenia, drowsiness, vertigo, depression, paresthesia, hypotension, angina pectoris, palpitations, chest pain, abnormal ECG DDI: Serious Antidiabetics causes hypoglycemia, Lithium toxicity occurs Diet: Without food Monitor: BP, potassium levels, RFT, LFT |
