Drugs Class ::

 Action Relaxes smooth muscle of venous and arterial vasculature.

Indications for Drugs ::

 Indications Relief of angina pectoris.

Drug Dose ::

 Route/Dosage

ADULT: Inhalation 0.3 ml prn. 1 to 6 inhalations from 1 capsule are usually sufficient. May be repeated in 3 to 5 min.

Contraindication ::

 Contraindications Hypersensitivity to nitrates; pregnancy; severe anemia; closed-angle glaucoma; orthostatic hypotension; head trauma; cerebral hemorrhage.

Drug Precautions ::

 Precautions

Pregnancy: Category X. Lactation: Contraindicated. Children: Safety and efficacy not established. Angina: May aggravate angina caused by hypertrophic cardiomyopathy. Drug abuse: May be abused for sexual stimulation or for effects of lightheadedness, dizziness, and euphoria. Glaucoma: May increase intraocular pressure. Orthostatic hypotension: May occur even with small doses; alcohol accentuates this reaction. Withdrawal: Dose is gradually reduced to prevent withdrawal reaction.

 Adverse Reactions

CV: Tachycardia; palpitations; hypotension; syncope; arrhythmias; edema. CNS: Headache; apprehension; weakness; vertigo; dizziness; agitation; insomnia. DERM: Cutaneous vasodilation with flushing. EENT: Blurred vision. GI: Nausea; vomiting; diarrhea; dyspepsia. GU: Dysuria; impotence. HEMA: Methemoglobinemia; hemolytic anemia. RESP: Bronchitis; pneumonia. OTHER: Arthralgia; perspiration; pallor; cold sweat.

 Action Relaxes smooth muscle of venous and arterial vasculature.

Drug Interactions ::

 Interactions

Alcohol: Severe hypotension and cardiovascular collapse may occur. Aspirin: Increased nitrate concentration and actions may occur. Calcium channel blockers: Symptomatic orthostatic hypotension may occur. Heparin: Effects of heparin may be decreased.

Drug Assesment ::

 Assessment/Interventions

  • Obtain patient history, including drug history and any known allergies.
  • Assess current status of cardiac, renal and hepatic function.
  • Assess vital signs, especially BP and pulse. Also assess for signs of tolerance to or abuse of the product.
  • Evaluate relief of symptoms and monitor for any adverse effects such as GI symptoms, headache, tachycardia, postural hypotension, skin flushing, rash, or possible arrhythmia.
  • Note any changes in serum cholesterol levels.

OVERDOSAGE: SIGNS & SYMPTOMS
  Severe headache, severe hypotension, flushing, tachycardia, vertigo, confusion, syncope, nausea, slow breathing or dyspnea, cyanosis, metabolic acidosis, convulsions, coma, death

Drug Storage/Management ::

 Administration/Storage

  • Help patient into sitting or reclining position.
  • Crush capsule, wave under patient’s nose and instruct to breathe deeply; 1 to 6 inhalations are usually sufficient. May repeat in 3 to 5 min.
  • Keep capsules in original container and keep container tightly closed.

Drug Notes ::

 Patient/Family Education

  • Caution patient that this medication must not be taken during pregnancy or when pregnancy is possible. Advise patient to use reliable form of birth control while taking this drug.
  • Advise use of acetaminophen for relief of headache.
  • Show patient how to crush capsule. Remind patient not to remove cloth-like covering.
  • Show patient how to keep record of usage, including frequency, dosage, and level of pain relief.
  • Explain potential for abuse.
  • Instruct patient to report these symptoms to physician: Blurred vision, dry mouth or persistent headache occurs or symptoms are not relieved.
  • Caution patient to avoid sudden position changes to prevent orthostatic hypotension.
  • Instruct patient to avoid intake of alcoholic beverages or other CNS depressants and aspirin.
