OXALIPLATIN
ALKYLATOR | ANTI-NEOPLASTIC
PK: D: 440 L (Vd) M: Nonenzymatic E: Urine (~54%), feces (~2%)

Indications & Dose: ADVANCED CANCER OF COLON/RECTUM Combination therapy IV Adult Inf: On day 1, 85mg/m2 oxaliplatin & 200mg/m2 leucovorin given simultaneously over 2h, followed by 5-fluorouracil (400mg/m2 bolus over 2-4 min) then 600mg/m2 of 5-fluorouracil over 22h as continuous infusion. On day 2, 200mg/m2 leucovorin infused over 2h, followed by 5-fluorouracil (400mg/m2 as bolus over 2-4min) then 600mg/m2 of 5-fluorouracil 22h as continuous infusion. Regimen may be repeated q2wk until disease progression/ unacceptable toxicity | STAGE III COLON CANCER Combination therapy/Adjuvant therapy IV Adult 85mg/m2 q2wk for 12 cycles with fluorouracil/leucovorin

Contra: Hypersensitivity, patients with pre-existing sensory neuropathies/myelosuppression, severe renal impairment

Precautions: Moderate renal impairment, elderly

ADR: Serious: Neurotoxicity, peripheral neuropathy, pulmonary fibrosis, peripheral edema, blood disorders, neutropenia, hypersensitivity reactions, hypomagnesemia, interstitial lung diseases, arthralgia, thromboembolism, AST, ALT, total bilirubin increased, Others: cough, dyspnea, dysuria, URI, rhinitis, epistaxis, pharyngitis, pharyngolaryngeal dysesthesia, allergic reactions, tubular necrosis, hypoxia, fatigue, fever, pain, headache, insomnia, nausea, vomiting, diarrhea, abdominal pain, constipation, anorexia, stomatitis

DDI: Serious Tacrolimus adverse/toxic effects may enhances, Irinotecan cholinergic toxicity enhances, Fluorouracil level moderately raises

Monitor: CBC with differential, blood chemistries includes Se Cr, ALT, AST & bilirubin, INR & PT, signs of neuropathy, hypersensitivity & respiratory effects
