Indications & Dose: COMPLICATED INFECT IN SERIOUSLY-ILL PATIENTS IV Adult Loading dose 25-30mg/kg may be used to achieve target concentration, then 15-20mg/kg/dose q8-12h | ENTEROCOLITIS PO Adult 500mg-2g/day in 3-4 divided doses 7-10 days, usual dose 125-500mg q6h Child >1 month: 40mg/kg/day in 3-4 divided doses added to fluids 7-10 days, max 2g/day | HOSPITAL-ACQUIRED PNEUMONIA IV Adult 15-20mg/kg/dose q12h | MENINGITIS IV Adult 30-60mg/kg/day in divided doses q8-12h or 500-750mg q6h with third-generation cephalosporins Child >1 month: 15mg/kg q6h | IT Adult 5-20mg/day Child >1 month: 5-20mg/day | PROPHYLAXIS AGAINST INFECTIVE ENDOCARDITIS Combination therapy IV Adult Dental, oral/upper respiratory tract surgery: 1g administered 1h before surgery, GI/GU procedure: 1g vancomycin with 1.5mg/kg gentamicin administered 1h prior to surgery Child Dental, oral/upper respiratory tract surgery: 20mg/kg/dose administered 1h prior to procedure, GI/GU procedure: 20mg/kg vancomycin with 1.5mg/kg gentamicin administered 1h prior to surgery | SUSCEPTIBLE GRAM-POSITIVE INFECT IV Adult 15-20mg/kg/dose, usual dose 750-1500mg q8-12h Child >1 month: 10mg/kg q6h

Precautions: Renal impairment, patients with inflammatory GI disorders, avoid rapid IV administration of drug, elderly, avoid in patients with history of impaired hearing, concurrent use of nephrotoxic or ototoxic drugs

ADR: Serious: Anaphylaxis, ototoxicity, thrombocytopenia, renal failure, interstitial nephritis, hypotension, vasculitis, Others: Nausea, vomiting, taste disturbances, chills, drug fever

DDI: Serious Ketorolac acute renal failure & GI bleeding, Neuromuscular blockers blocking activity prolonged, Digoxin half-life prolonged, Ciclosporin/Aminoglycosides/Zidovudine leads to risk of nephrotoxicity

Diet: With food

Monitor: Periodic renal function tests, urinalysis, WBC, Se trough vancomycin concentrations
