Indications & Dose: ACNE VULGARIS TP Adult Solution/gel/ointment (2%): Apply a thin film over the cleansed & affected area in morning & evening | CHANCROID PO Adult 500mg TID/QID 1 wk | CHLAMYDIAL INFECT PO Adult 500mg QID 1 wk Child <45 kg: 50mg/kg/day in 4 divided doses 2 wk | INFECT PO Adult Severe: up to 4g/day Child Mild/moderate: 30-50mg/kg/day in divided doses q6-12h | Severe IV Child 15-50mg/kg/day, max 4g/day | LEGIONNAIRES’ DISEASE PO Adult 1.6-4g/day in divided doses 3 wks | LYME DISEASE PO Adult 500mg QID 2-3 wk | LYMPHOGRANULOMA VENEREUM PO Adult 500mg QID 21 days | NEONATAL GONOCOCCAL/CHLAMYDIAL CONJUNCTIVITIS Prophylactic therapy TP Child Ointment: 0.5-1cm ribbon instilled into each conjunctival sac | NON-GONOCOCCAL URETHRITIS/ GONORRHEA PO Adult 500mg QID 1 wk | PERTUSSIS PO Adult 500mg (base) QID 2 wk Child 40-50mg/kg/day in 4 divided doses 2 wk, max 2g/day | PHARYNGITIS/TONSILLITIS PO Adult 500mg BID 10 days Child 20mg/kg/day or 40mg/kg/day (as ethylsuccinate) in 2 divided doses 10 daysstreptococcal | PNEUMONIA PO Child Infants: 50mg/kg/day in 4 divided doses 2 wk | PREOP BOWEL PREPARATION PO Adult 1g at 1, 2 & 11 PM on the day before surgery combined with mechanical cleansing of the large intestine and oral neomycin Child 20mg/kg at 1, 2 & 11 PM on the day before surgery combined with mechanical cleansing of the large intestine and oral neomycin | RECURRENT RHEUMATIC FEVER Prophylactic therapy PO Adult 250mg BID | SUPERFICIAL INFECTIONS OF CONJUNCTIVA/CORNEA TP Adult Ointment: Instill ½ inch 2-6 times/day based on severity of the infect Child Same as adult dose | SYPHILIS PO Adult 30-40g in divided doses 10-15 days

Contra: Hypersensitivity, porphyria

Precautions: Hepatic impairment, myasthenia gravis, use of lactobionate in patients with severe renal impairment, QT interval prolongation agents, history of arrhythmias

ADR: Serious: Torsade de pointes, arrhythmias, hepatic dysfunction, hepatitis, ototoxicity, aggravation of myasthenia gravis, Others: allergic reactions, nausea, vomiting, diarrhea, abdominal pain/cramps, rash, increased appetite, urticaria

DDI: Serious Corticosteroids increases both its therapeutic & adverse effects of drug, Simvastatin leads to acute rhabdomyolysis, Antihistamines leads to life-threatening torsade de pointes arrhythmias, Bromocriptine/Benzodiazepines/Buspirone/Carbamazepine/CCBs/Ciclosporin/Digoxin/Disopyramide/Quetiapine/Theophylline levels increases, Vinblastine toxicity increase, Ergotamine toxicity raises
