Indications & Dose: ATOPIC ECZEMA TP Adult Ointment (0.03/0.1%): Apply twice daily Child 2-16 yr: Ointment (0.03%): Apply twice daily up to 3 wk, then reduce to once daily until lesion clears, >16 yr: Same as adult dose | CARDIAC ALLOGRAFT TRANSPLANTATION PO Adult 75µg/kg/day in 2divided doses q12h | IV Adult Inf: 10µg/kg/day after transplantation of 6h | CROHN’S DISEASE PO Adult 200µg/kg/day in 2divided doses 10wks | HEPATIC ALLOGRAFT TRANSPLANTATION PO Adult 100-150µg/kg/day in 2divided doses q12h Child 150-200µg/kg/day in 2divided doses q12h | IV Adult Inf: 30-50µg/kg/day after transplantation of 6h | RENAL ALLOGRAFT TRANSPLANTATION PO Adult 200µg/kg/day in 2divided doses q12h | IV Adult Inf: 30-50µg/kg/day after transplantation of 6h |

Precautions: HTN, myocardial hypertrophy, renal/hepatic disease, pediatrics, DM, hyperuricemia, lymphoma ADR: Serious: HTN, ARF, nephropathy, DM, arthralgia, albuminuria, hypokalemia, renal tubular necrosis, pericardial effusion, Others: diarrhea, abdominal pain, nausea, vomiting, constipation, anorexia, dyspepsia, hyperlipemia, hyperkalemia, hypomagnesemia, insomnia, fever, headache, edema, acne, alopecia, wound healing impaired, hirsutism, skin disorder, photosensitivity reaction, mood changes, sleep disturbances, blood dyscrasias DDI: Serious Cimetidine/Chloramphenicol/Clarithromycin/Clotrimazole/Danazol/Erythromycin/Fluconazole/Ketoconazole/Nifedipine increase drug level, HIV protease inhibitors/Antacids/Hormonal Contraceptives/PPIs increase drug levels, Sirolimus increases risk of hepatic artery thrombosis, graft loss, death, Ciclosporin increases risk of nephrotoxicity, Spironolactone/Amiloride/Triamterene leads to severe hyperkalemia, Phenytoin/Carbamazepine/Rifampicin levels are increased, Vaccines (Live & inactivated) response is abolished Diet: Before food Monitor: Renal function, LFT, serum electrolytes, glucose,vital signs, signs & symptoms of anaphylactic reactions |
