Drugs Class ::

Indications for Drugs ::

 Indications Treatment of invasive aspergillosis in patients refractory to or intolerant of other antifungal therapies.

Drug Dose ::

 Route/Dosage

ADULTS: IV EMPH: Loading dose: 70 mg given by slow infusion of » 1 hr on day 1. Maintenance dose: 50 mg daily by slow infusion of » 1 hr thereafter.

Drug Side Effects ::

 Adverse Reactions

CV: Vein complications; phlebitis; thrombophlebitis; tachycardia; vasculitis. CNS: Headache; insomnia; paresthesia; tremor. DERM: Flushing; erythema; induration; pruritus; rash; sweating. GI: Nausea; vomiting; abdominal pain; anorexia; diarrhea. HEMA: Eosinophilia; anemia; decreased hematocrit, hemoglobin, neutrophils, WBC count, and platelet count; increased prothrombin time. HEPA: Increased bilirubin, ALT, and AST. META: Edema; increased serum bicarbonate, potassium, and uric acid. OTHER: Myalgia; back pain; musculoskeletal pain; asthenia; fatigue; fever; chills; flu-like illness; malaise; pain; warm sensation; anaphylaxis; hypersensitivity; tachypnea.

Drug Interactions ::

 Interactions

Cyclosporine: Avoid concurrent use if possible, because caspofungin levels may be elevated, increasing the risk of side effects. Inducers or mixed inducers/inhibitors of drug clearance (eg, carbamazepine, dexamethasone, efavirenz, nelfinavir, nevirapine, phenytoin, rifampin): Caspofungin blood levels may be decreased, reducing the efficacy. Tacrolimus: Tacrolimus blood levels may be decreased, reducing the efficacy. INCOMPATIBILITIES: Do not mix or coinfuse with other medications.

Drug Assesment ::

 Assessment/Interventions

  • Obtain patient history, including drug history and any known allergies. Note history of liver disease.
  • Complete CBC and blood chemistry tests prior to initiating therapy.
  • Obtain fungal culture as needed before onset of therapy.
  • Monitor IV infusion site for signs of reaction or phlebitis.
  • Assess patient for CNS, GI, cardiovascular, hematological, musculoskeletal, and dermatologic side effects. Notify physician if these effects occur.
  • Monitor laboratory values, including liver enzymes and serum potassium during therapy.
  • Monitor temperature during and after administration. Fever is common.
  • Notify physician if severe side effects or signs of anaphylactic reaction occur; stop infusion.

Drug Storage/Management ::

 Administration/Storage

  • Follow manufacturer’s instructions for reconstition and administration. Use 0.9% Sodium Chloride Injection. Do not reconstitute with diluents containing glucose.
  • Allow refrigerated vial of caspofungin powder to come to room temperature before reconstituting.
  • Do not administer reconstituted solution directly into patient. Reconstituted solution must be transferred to IV bag or bottle containing 0.9% Sodium Chloride for Injection.
  • Use aseptic technique while handling medication.
  • Do not mix or coinfuse with other medications.
  • Do not administer if particulate matter, discoloration, or cloudiness are noted.
  • Administer once daily by slow IV infusion of » 1 hr.
  • Store unused vials in refrigerator. Reconstituted solution may be stored at temperature < 77°F for up to 1 hr prior to preparation of patient infusion solution. Final patient infusion solution in the IV bag or bottle can be stored for up to 24 hr at a temperature < 77°F.

 Patient/Family Education

  • Explain name, dose, action, and potential side effects of drug.
  • Instruct patient to report any discomfort at injection site immediately.
  • Instruct patient to report any of the following symptoms to the physician: fever, chills, nausea, or headache.
