Precautions: hepatic dysfunction, on nephrotoxic drugs (aminoglycosides, cisplatin, loop diuretics, ciclosporin, and drugs that are secreted by renal tubules, such as probenecid, penicillin)

ADR: Serious: ARF, MI, cerebrovascular events, increased ALT, AST, anemia, leukopenia, neutropenia, Others: edema, hepatitis, colitis, intestinal pneumonitis, fatigue, rash, nausea/vomiting, stomatitis

DDI: Others NSAIDs/Aspirin in high doses decreases pemetrexed elimination

Monitor: CBC with differential & platelets, serum creatinine, BUN, total bilirubin, ALT, AST
